As stated above, a single physician or other clinician cannot possibly address all needs of people with chronic diseases. That system results in fragmented, substandard care that is not patient centered and delivers inferior clinical and financial results. I have heard dozens of individual clinicians complain that their patients are not compliant. “I prescribe the right medicines. I tell them the right thing to do. They just don’t do it. Why should I be held accountable when they don’t follow my advice?”
