Let me begin by thanking you for being engaged with this daily content on optimal medical therapy to slow aging and delay chronic disease. We have developed another resource to provide training and certification to prepare you to function on a medical home team that produces optimal medical therapy (OMT) consistently. The material on this site and the CME complement each other to close the gap between knowledge and practice. The CME provides a base of knowledge and this site updates it. As new information comes up it will be added to this enduring CME material online.
Here is the link to Congruity Health University. It is an educational program to bring all members of your Chronic Cardiometabolic Condition Clinic™ team onto the same sheet of music. It will help them understand why optimal medical therapy (OMT) is important and how to produce it consistently. The courses begin with coronary artery disease, which is the highest risk, highest cost condition and are presented in order of risk and cost. That enables your team to show the most rapid and impressive clinical and financial benefits.
At the end of the course content, there will be a test. Those who pass the test will receive a basic certification as a Chronic Cardiometabolic Condition Clinic™ team member. When graduates achieve OMT in 60% of their patients with diabetes and vascular disease, they will receive advanced certification. Clinics who achieve OMT to that level can also receive advanced certification which is a way to assure healthcare purchasers that you produce the highest value cardiovascular care.
