The legacy unacceptable fee-for-service is dead, and has been dead for a long time in my book. Medical homes supported by a trusted caregiver team in person or remotely will be the future, like it or not. It’s already here.

This is invaluable information that providers can/should be using consistently but don’t. How to institutionalize this practice as SOC?

Optimal medical therapy for stable coronary artery disease is a universal standard of care that is to be used prior to coronary artery disease as specified by new guidance from the American College of Cardiology. Despite this standard, no one is teaching it, and it is not available for most patients. Influential stakeholders like you and others in the Moving to Value Alliance will be essential in pushing for implementation. We could improve the care for thousands of patients in a matter of months.

The glaring lack of attention to the lifestyle metabolic dysfunctions and insulin resistance/endothelial dysfunction/microvascular disease which are by far the most causes of chronic cardiovascular disease is the root cause of the systemic failure. Fundamentally change of the system must happen to finally stop this unfortunate injustice.

The fee for service system is only dead to us. It continues to be the dominant payment methodology in the US and the biggest barrier to better health at lower cost.

It is a rigged game, corrupt to its core. We ALL know that. I left that kabuki dance theater in 1997 and embarked on the journey to create something worthy my time and energy and to save myself from the soul crushing oppressive regime - the Late Stage Sickness Seeking Profiteering Monopolistic Legacy American Medical Industrial Complex Kabuki Dance Theater!

It’s a form of slavery of a brainwashed population, tethered to a corrupt system. People are awakened, and walking away from this failed legacy system. More than 50% are refusing to play this game anymore.
