Only 40% of those who live in North America trust information from healthcare workers. That is a huge barrier when it comes to managing chronic diseases. Successful management of those conditions requires long-term trusting relationships. That is why value-based payment systems and chronic condition teams are so critical. Our fee-for-service primary care system pays providers for moving quickly from room to room and creating a note to document the visit. That model does not support trusting relationships. It does not pay providers to achieve results. As a consequence, only 44% of Americans with high blood pressure have control to less than 140/90 and control is getting worse. Kaiser Permanente achives a blood pressure control rate of 90% by using protocols, systems, and non-physician health workers. Thousands of patients have a stroke in this country annually because we have not made the changes that are called for.
This entire site reviews the disconnect between the science of chronic disease and practice in the real world. The new science is more promising than ever before. There is a mountain of evidence supporting this idea. We can have longer healthier life at lower cost. We can have patient-centered care now.
