The United States has much higher rates of death due to coronary artery disease than the Europeans, and we spend more on healthcare. Optimal medical therapy first for stable angina delivered by primary care with minimal testing has been the standard in Great Britain for at least a decade. Most primary care practices in the US are not set up to deliver OMT. The image above comes directly from the British guideline. Please help me understand why we are not adopting this evidence.
The 10 year, $100 million dollar “Ischemia” trial data showed that not only there was absolutely “zero” difference in the ultimate outcomes between the medically treated arm and coronary artery interventions arm of the trial; but also there were 13-15% patients suffered major adverse cardiac events, including deaths from both arms of this trial. They failed to look at populations outside of these “established protocols and guidelines” and frankly flopped! So, even treated by the “best” from the “best institutions”, people still die. Something is terribly missing here…
