Thanks for useful tables. Curious - where are conditions like Parkinson's Disease?

They are common and devastating as you know. They are just less common than the type 21. I am more and more convinced that excess oxidative particle generation is a common denominator in aging and developing chronic illnesses more rapidly, like Parkinson's.

Thanks for the reply and the link - I will read with interest. On a similar theme, I believe Prof Naviuax's "Cell Danger Response" is involved in conditions like PD, have you come across this before? http://www.outthinkingparkinsons.com/articles/cell-danger-response

Thanks Gary. I reviewed this in detail. I think the speaker is saying that a drop in ATP from the mitochondria is a danger response. It can occur in settings like starvation or low oxygen supply. It activates AMPK which is a key factor in precision medicine and that is a central consideration in all the discussions on my site. Medications like empagliflozin for diabetes activate AMPK also which explains why they lower hospitalizations for congestive heart failure by 30% whether the patient is diabetic or not!

Would earlier diagnosis impact cost of care. For example, the largest cost; diabetes?

Great question. You can show the greatest impact quickly on patients with patients with diabetes and a complication like chronic kidney disease. The earlier you start optimal medical therapy the better. In the Steno two trial, at 8 years the difference between usual care and OMT was so great, everyone went on OMT. Even so, because of metabolic memory, the OMT group continued to do much better. If you start in prediabetes you can slow progression dramatically and delay cost

It’s hard to watch patients having to make care choices based on costs. Hopefully the legislation President Biden has been so passionate about will lead to the changes needed.
